OVERVIEW
Impingement has a signature you can almost map. Reach for a seatbelt, and it's there. Wash your hair, and it's there. Put a plate in an upper cabinet, tuck in a shirt behind your back, and there it is again. The common thread is the arm working at or above shoulder height, which is exactly the position that pinches the irritated tendons.
Inside the shoulder, your rotator cuff tendons pass through a tight space at the top of the joint. Raise the arm and that space narrows. Irritated tendons get squeezed against the bone above, they swell, and the swelling crowds the space further. Left alone, it feeds itself and slowly gets worse.
Most impingement, though, responds well to the right care without surgery. At Modern Medicine of South East Tucson, AZ, we work on the mechanics behind the pinch, so the shoulder opens back up and stays that way.

Answering Your Questions
Which movements tend to set it off?
Anything at or above shoulder height is the usual culprit. Reaching into a cabinet, washing your hair, putting on a jacket, lifting overhead. Those all narrow the space where the tendons sit, so they pinch. Knowing your triggers helps us pick what to ease off early.
Is impingement the same as a rotator cuff tear?
Not the same, but related. Impingement is the tendon getting pinched and irritated. A tear is actual damage to it. Impingement left long enough can wear into a tear, which is one reason to deal with it early. We check which one you've got.
Why does the shoulder feel weaker, not just sore?
When the tendons are irritated and pinched, the muscles around the shoulder don't fire as well, so the arm feels weak on top of painful. As therapy calms the irritation and rebuilds the cuff, that strength usually comes back.
Can I keep exercising while it settles?
Usually, with swaps. Overhead pressing and certain lifts tend to aggravate it, so we route around those for a while and keep the rest. You don't have to shut everything down, just ease off what feeds the pinch.
How much does posture really matter?
More than most people think. Rounded, forward shoulders narrow the very space the tendons pass through, so every reach pinches a little. Opening that posture back up takes pressure off the tendons, which is why it's a real part of the plan.
