There's a specific arc where it hurts. Down low, the arm's fine. Up high, past the top, sometimes fine again. But right around shoulder height, coming up or going down, there's a sharp catch. That painful middle band is a hallmark of shoulder impingement.
Most people notice it creeping in over weeks. A twinge reaching for a high shelf, then every overhead move, then it's waking them at night.
What's Getting Pinched
Your rotator cuff tendons pass through a narrow space at the very top of the shoulder. Raise your arm and that space tightens. If the tendons are already irritated, they get squeezed against the bone above them, and they swell, which crowds the space even further. That's the loop that makes it worse the longer it's ignored.
It's rarely one injury. Posture, repetition, and muscle imbalance stack up until the shoulder finally speaks up.
Why Therapy Works For Most Cases
The pinch is a mechanics problem at heart. How your shoulder blade moves, how the cuff fires, how your posture sets the whole thing up. Change the mechanics and that space opens back up on its own.
Surgery addresses the tendon. Therapy addresses why the tendon got pinched to begin with. For most impingement, going after the cause is what actually holds.
How We Treat It
We calm the pain first so you can move, then we rebuild:
- Rotator cuff work to keep the ball centered in the socket
- Shoulder blade strengthening for a stable base
- Stretching the tight areas, often the back of the shoulder and chest
- Posture correction, since desk work feeds this hard
- A planned, gradual return to overhead activity
We'll also tell you what to lay off for a bit. Two weeks away from one repeated motion can change the whole picture.
Don't Let It Grind Into A Tear
Here's the reason not to just wait it out. Impingement that drags on keeps grinding those same tendons, and grinding tendons can wear into an actual tear over time. What could've been calmed in a few weeks becomes a bigger repair. Early attention usually means a simpler fix.
Getting Your Range Back
By the time impingement's been hanging around, the shoulder often stiffens and loses motion. Part of the work is winning that range back, not just quieting the pain. A shoulder that doesn't move well stays a vulnerable one, so we rebuild the motion alongside the strength.
