ABOUT SCIATICA TREATMENT
The first thing worth knowing about sciatica is that it isn't really a diagnosis. It's a description. The sciatic nerve starts in your lower back, passes through the buttock, and runs down the back of your leg. Irritate or compress it anywhere along that route and the pain radiates along the whole length of it.
That's why your calf can be screaming when the actual trouble sits in your spine. It's also why massaging the leg accomplishes nothing.
How people describe it varies quite a bit. Some say electric, some say burning, and some get numbness or tingling rather than outright pain. The near-universal complaint is that sitting makes it worse, which turns commutes and desk days into something you dread.
What's irritating the nerve differs from person to person. A disc pressing on the nerve root, narrowing in the spine that leaves the nerve less room, or a tight muscle deep in the buttock crowding it. Which one you've got shapes the treatment, so figuring that out comes first.

Answering Your Questions
Why does my leg hurt if the problem is in my back?
That nerve runs the entire distance, so trouble at the top gets felt all the way down. Something pinches it near the spine and your brain gets the report from wherever along the nerve it lands, usually your calf or the back of your thigh. Chasing the leg never works. You've got to deal with what's bothering the nerve up top.
Should I just rest until it settles?
A bit early on, sure, but not for long. Lying still seems like the obvious move when your leg is on fire, except overdoing it works against you. Your back stiffens, the muscles supporting it go soft, and the whole thing drags out longer than it should have. We'll help you find how much movement is actually helping.
Why is sitting so much worse than standing?
Sitting puts more pressure on the discs in your low back than standing does, and a slouchy chair or a reclined car seat makes it worse. That's why desk days and long drives are rough. Fixing how you sit lifts a constant daily irritation off the nerve.
Will I end up needing surgery?
Most people don't. The large majority of sciatica calms down with conservative care given an honest shot. Surgery comes up mainly when there's real weakness developing, when symptoms keep getting worse, or when months of good treatment haven't moved anything.
How long before it goes away?
Depends on what's causing it. Some cases quiet down in a few weeks, stubborn ones take a couple of months of steady work. Sticking with the exercises is what speeds it up, and we adjust the plan as your symptoms change.
