ABOUT SHOULDER IMPINGEMENT

Reaching for a mug on the top shelf shouldn't stop you cold. With shoulder impingement, it does. The pain shows up when you lift your arm to about shoulder height, and it can nag you at night when you roll onto that side. It builds slowly, too. One week it's a small twinge reaching for something high. A month later you're avoiding half the movements you used to do without thinking about them.

Shoulder impingement happens when the rotator cuff tendons get pinched in the tight space at the top of your shoulder. Every time you raise your arm, that space narrows and the tendons rub. Do it enough and they get irritated and swollen, which makes the pinching worse. Overhead work, sports like swimming and tennis, and years of desk posture all feed into it. It's rarely one injury. It's usually repetition and muscle imbalance stacking up until the shoulder starts complaining.

The good part is that most impingement gets better without surgery. At Modern Medicine of Northwest Tucson, AZ, we treat the cause, not just the sore spot, so your shoulder moves freely again and stays that way.

Answering Your Questions

Do I need surgery for shoulder impingement?

Not usually. Most cases settle down with therapy that fixes the mechanics behind the pinching. Surgery should always be treated as a last resort.

Why does it hurt more at night?

Lying on the shoulder compresses the irritated tendons, and there's nothing to distract you from the ache. A lot of people notice the pain most when they're trying to sleep.

How long until I feel better?

Many people feel real improvement in four to six weeks. Full strength takes longer, but daily activities usually get easier well before that time.

Can I keep working out?

Often yes, but you might need to make some changes. We'll tell you what to avoid for now and what's safe. We want you to be able to stay active without feeding the problem.

Will it come back after it heals?

It can if the habits that caused it don't change. That's why we fix the mechanics and posture behind the pinching. Fixing needs to go beyond just managing pain so the relief is long-term.
